Hi! It’s a pleasure to have you here! If you’ve already completed your interview and are excited to get started as a CogniTutor, that’s fantastic! We do need to have your information on file for you to proceed with being assigned students to tutor.

Required Steps – List

To ensure you’re ready to begin tutoring and receive payments, please complete all the steps

Each step is crucial, so make sure to provide all necessary details, particularly in the background check and Payment Setup Form. Get these completed, and you’ll be all set to start!

    1. BACKGROUND CHECK

    Process:
    1. Find a location nearest you that is a LiveScan location.
    2. Call and make an appointment
    3. When you show there, make sure to let them know you are needing a LiveScan Level 2 Background Check and Fingerprinting done for CogniTutor and give them the ORI number (V53030062)
    4. If they have any issues, have them call us. We can be reached at 813-798-7441.
    5. sign this waver and send it to CogniTutor, either digitally using your preferred PDF editor support@cognіtutor.com, or printed and mailed to CogniTutor, 325 Sand Pine Trail, Winter Haven, FL 33880
    Results typically take 48 hours. Your education specialist will contact you when received.

    3. LEARNSPEED SETUP

    You received an email inviting you to set up your profile on LearnSpeed, which is our management software that will make your job as a tutor easier. Learnspeed:

    1. Provides tutors calendars that must be kept up-to-date (Put your student sessions in the calendar!)
    2. Sends tutors reminders about their sessions
    3. Sends session reminders to student/parent
    4. Where tutors input session report (this is how you are paid) (download the assessment form, fill out, and send back in an email)
    5. Receives payments from parents

    4. PAYMENT AGREEMENT FORM

    Please read and review the AUTHORIZATION AGREEMENT before signing.
    Please type your name here as the electronic signature of the Prospective Tutor.

    From here onward the form will ask for a signature and it will be that of the Prospective Tutor.

    5. PAYMENT SETUP FORM

    $0.50 will be deducted for each direct deposit transaction. Payments can take up to 7 business days to show in your account. Timing varies depending on your financial institution. Please provide your bank routing number and account number by clicking on the Payment Setup Form link below.

    6. TAX INFORMATION FORM

    Please read and review the TAX INFORMATION AGREEMENT before signing.

    CogniTutor needs some information. As a 1099 contractor with CogniTutor, we need some information from you for tax purposes. You will soon receive an email from CogniTutor (via QuickBooks) with the sender address [email protected]. Please click on the green "Get Started" button in the email and follow the steps to complete and submit your W-9 form.

    7. TERMS AND CONDITIONS

    Please READ and ACCEPT the terms BELOW before submitting the form.

    Please read and review the INDEPENDENT CONTRACTOR AGREEMENT FOR TUTOR and PRIVACY POLICY before signing.

    By typing in Tutor’s name and date below, Tutor affirms that he or she has read and understood the Independent Contractor Agreement for Tutor and Privacy Policy and agrees to and accepts all of the terms and conditions contained therein.

    I understand that CogniTutor monitors in-person and online sessions by speaking frequently with guardians, students, and more. If an independent contractor working with CogniTutor is falsifying time met with students, CogniTutor may be forced to take legal action.
